To save time on the day of your appointment,
please download these forms.
Fill them out and bring them with you to your appointment.
All patients should also bring the following to their appointment: a photo ID, their insurance card, a list of their current medications, and any glasses they use.
We also recommend you bring any previous medical (eye) records and test results.
You can use our Records Release Form to request records from your previous doctor be sent to Kalyani Eye Care.

Insurance/Referral
We accept Medicare as well as most major commercial insurance plans. We ask that you bring your insurance card(s) with you to all appointments so that we can bill the insurance company properly. If your medical insurance requires a co-pay, you will be asked to pay this at the time of your visit. For your convenience, we accept cash, checks, and credit cards.
Some insurance plans require that you have a referral from you primary care provider. We ask that you bring this with you. Please inform our staff when you make your appointment if you need any assistance in obtaining a referral. We will be happy to help you with this.
